The Southwest Ohio Regional QI Hub is Seeking Members for its Patient and Family Advisory Council (PFAC)
WHAT
Your primary care clinic is recruiting patients and family members to help improve hypertension care in Southwest Ohio by sharing feedback on your experience.
WHO
You or your family member who has hypertension or heart disease and is enrolled in Medicaid or a Medicaid managed care plan.
HOW
Healthcare providers and health plans will work with you to learn about your needs and experience and make positive changes in care for you and your loved ones.
WHEN
We will meet every other month to get feedback on care materials and Ohio Medicaid providers management of hypertension care. Participation spans from November - September. You will be compensated $25 for each meeting.
